Discharge PlanningDischarge Planning is a service that is available at Pender Community Hospital. If you need assistance planning for your care after dismissal, the Discharge Planner will visit with you about your needs. The Discharge Planner can help you make arrangements for Home Health Care, Hospice, or medical equipment to make the transition from hospital to home as smooth as possible. The Discharge Planner may also assist you in making arrangements for skilled care or long-term care if needed. The Discharge Planner can also help you with questions you may have about insurance or Medicare coverage.
If you have questions about planning for your discharge or about your health care coverage, you may contact the Discharge Planner by calling extension 4016, or notifying your nurse.
Your DismissalWhen your doctor gives the order for your dismissal, instructions and/or prescriptions will be left at the nurse's station. Your nurse will give you instructions about your care after discharge. The nursing staff will also help you arrange for your departure and accompany you to your transportation. If you have any questions about your medications or instructions on what to do at home, please ask the nurse before you leave. All patients (or a family representative) are asked to stop at the Business Office at the time of discharge to verify insurance information and complete financial arrangements.
Dismissal ChecklistWhen you leave, be sure to check the following:
Do you have all of your personal belongings?
Are valuables picked up from the safe?
Did you pick up any medications brought from home that may have been left at the nurse's station?
Have you checked with the nurse's station for instructions or medications to take home, and do you understand them?
Did you check with the Business Office to verify insurance information and complete financial arrangements?
Have you left a forwarding address?
